This 43-years old man had a severe episode of myocardial infarction. He had multi-vessel disease three years ago and following the angiogram, he underwent an aneurysectomy for an LV aneurysm. For the last two years, he would have episodes of LV failure, at least twice a month. A cardiac MRI was performed to assess the extent of non-infarcted myocardium and function.
The MRI showed the small LV following the aneurysectomy (cine file). Virtually all segments showed >75% infarction and his EF was 10%. He succumbed to LV failure three days after the MRI.
