As against the previous case, where the RCA showed a clear-cut origin from the left sinus, is this one, where the RCA on the VRT image (Fig. 1) looks like it has a malignant course. However on carefully evaluating the MIP image, you can clearly see that the RCA arises from the right sinus and that it just has a more leftward origin (Fig. 2). Also, there is no compression or narrowing of the proximal RCA.
