As mentioned in the last post, CMR/MRI can be used to diagnose dilated cardiomyopathy. Two excellent publications, by Moon J et al in Circulation and Soriano CJ et al in JACC have shown that the absence of delayed hyperenhancement or just mid-myocardial enhancement in patients who have diffuse hypokinesia, is highly suggestive of dilated cardiomyopathy and rules out "ischemic" cardiomyopathy / cardiomyopathy due to coronary artery disease.
This patient has diffuse hypokinesia of all segments (cine file) and complete absence of delayed hyperenhancement, as seen on one representative mid-cavitary, short-axis image.
