Sarcoidosis

This 35-years old man presented with ventricular tachycardia. After stabilization, an echocardiogram was performed, which showed wall motion abnormalities. A contrast enhanced MRI was then performed. The delayed enhanced images (Figs. 1-4) show patchy areas of mid-myocardial and epicardial enhancement. An axial image through the mediastinum shows subcarinal and bilateral hilar non-necrotic adenopathy. A short axis cine (28176 - sa 02.avi) study shows marked hypokinesia of all segments.

Sarcoidosis not uncommonly involves the myocardium and presents as patchy areas of enhancement on the delayed hyperenhanced images. Often there is other clinical and radiological evidence of sarcoidosis (nodes, lung changes, neurologic involvement, etc) as in this case.
